交換學生/研修生入學申請表

Application Form for Exchange Student / Short-term Study 

 

 

※上傳必備文件(JPG檔)

1) 兩吋免冠白底彩色證件照電子檔：照片尺寸4.5×3.5公分、人像自頭頂至下顎之長度介於3.2至3.6公分、背景白色、耳眉不可遮掩。
2) 在校歷年成績單正本1份：須有校方章戳之證明。
3) 在學證明：內容須含學生資料及修業時間，並蓋學校戳章。
4) 身分證彩色雙面掃描檔：請以彩色將正反面掃描在同一張上。

1. 請輸入欲申請修讀系所之完整正確名稱。【查詢】>教學單位
2a.若欲申請本校音樂學系，請加註主副修樂器。例如：音樂學系(主：鋼琴/副：長笛)，本校將依此開設主副修課，除因本校無該項樂器師資，否則不得修改。
2b.若欲申請本校藝術創意產業學系【碩士班】，請加註申請組別(視覺藝術教育、民族藝術)。例如：藝術創意產業學系(視覺藝術教育)
2c.若欲申請本校課程設計與潛能開發學系【碩/博士班】，請加註申請組別(教育、多元文化教育、科學教育)。例如：課程設計與潛能開發學系(多元文化教育)
